Your Responsibilities:
Patient Outreach & Support - Conduct monthly check-ins with patients, offering education and connecting them to necessary resources.
Care Plan Management - Develop and maintain individualized care plans to ensure patients receive timely and appropriate care.
Monitor Patient Progress - Track health status, provide proactive interventions, and prevent gaps in care.
EMR Review & Documentation - Assess electronic medical records (EMRs) to determine eligibility for care management services.
✅ Ensure Compliance - Follow Medicare, HIPAA, and healthcare policies while maintaining accurate documentation.
Performance & Outreach Goals - This is a phone-based role with a high volume of patient interactions. You'll be expected to meet daily performance targets, ensuring each patient receives a minimum of 20 minutes of care coordination per month.
What We're Looking For:
✔ Active LPN License in Massachusetts (or a Compact License with the ability to obtain MA licensure).
✔ 5+ years of patient-centered care experience - Strong clinical knowledge is essential!
✔ Tech-Savvy - Comfortable working with EMR systems and digital platforms in a remote setting.
✔ Excellent Communicator - Strong verbal skills and a passion for patient engagement.
✔ Self-Motivated & Organized - Ability to work independently and meet performance expectations.
✔ Bilingual (Spanish-English) Preferred - Fluency in Spanish is a plus!
